INSURANCE & PAYMENT

Three Birds Counseling is a private-pay, out-of-network practice. While we do not bill insurance companies directly, some insurance plans offer out-of-network benefits that may reimburse you for a portion of your therapy costs.

You are responsible for paying the full session fee at the time of your appointment. Upon request, we can provide an itemized receipt called a superbill, which contains information you may need to submit a claim to your insurance company for possible reimbursement.

Because coverage varies by plan, we encourage you to contact your insurance company before beginning services to understand your benefits and potential out-of-pocket costs.

QUESTIONS TO ASK YOUR INSURANCE COMPANY

  • Does my plan include out-of-network mental health or behavioral health benefits?

  • Do I have an out-of-network deductible? If so, how much is it, and how much have I already met?

  • Once my deductible is met, what percentage of out-of-network mental health services will my plan reimburse?

  • Is reimbursement based on the actual amount I pay or on the insurance company's allowed amount? What is that allowed amount for these services?

  • Is there a limit on the number of therapy sessions covered per year? Are there any other limits or requirements?

  • Does my plan cover the following CPT codes when services are provided by an out-of-network clinician?

    • 90791 — Psychiatric diagnostic evaluation

    • 90834 — Psychotherapy, approximately 50 minutes

    • 90837 — Psychotherapy, approximately 60 minutes

  • If I'm receiving telehealth services, are those same codes covered for virtual appointments? Does my plan require a particular telehealth modifier (such as 95) or place-of-service code?

  • Do I need prior authorization, a referral, or any other approval before seeing an out-of-network provider?

  • How do I submit an out-of-network mental health claim? What forms, documentation, or information are required?

  • Is there a deadline for submitting claims?

Please note: Out-of-network reimbursement is not guaranteed. Your insurance company determines eligibility, coverage, reimbursement amounts, and any applicable requirements. We encourage you to verify your benefits directly with your insurer.

A note about other services: Insurance coverage for couples counseling, parent coaching, consultations, and psychological or educational evaluations may differ from coverage for individual psychotherapy. If you're considering one of these services, we encourage you to ask your insurance company specifically about eligibility and reimbursement before scheduling.

CANCELLATION &
MISSED APPOINTMENT POLICY

We understand that schedules change and unexpected circumstances arise. At Three Birds Counseling, your appointment time is reserved specifically for you. When you need to cancel or reschedule, we ask that you provide as much notice as possible so that your appointment time may be offered to someone else.

We require a minimum of 48 business hours' notice to cancel or reschedule an appointment without incurring a fee. Weekends do not count toward the 48-hour notice period.

Appointments canceled with less than 48 business hours' notice, as well as missed appointments, are subject to the full scheduled session fee ($195 per hour for counseling appointments), except in the case of a documented, genuine emergency that prevents you from attending.

A few important things to know:

  • Appointment reminders are a courtesy. You are responsible for keeping track of your scheduled appointments, even if you do not receive a reminder.

  • Please leave a message if you call to cancel. A missed call without a voicemail does not constitute notice of cancellation.

  • Late cancellation and missed appointment fees are not eligible for insurance reimbursement and cannot be included on superbills.

  • Outstanding cancellation fees must be paid before your next counseling appointment. Repeated late cancellations or missed appointments may affect your ability to continue services.

We review this policy during the scheduling process and at the beginning of treatment so that expectations are clear from the start.

Example: Monday appointment

If your appointment is scheduled for Monday at 2:00 PM, you would need to cancel by Thursday at 2:00 PM the previous week to provide 48 business hours' notice, assuming weekends are excluded and there are no additional holiday closures.

GOOD FAITH ESTIMATE

Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use their insurance to pay for services have the right to receive a Good Faith Estimate explaining the expected costs of their care.

You have the right to receive a Good Faith Estimate in writing when scheduling eligible services or upon request. This estimate outlines anticipated charges and is not a bill.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute those charges through the federal patient-provider dispute resolution process.

We believe you deserve to understand the cost of your care before beginning treatment. Please contact us with any questions about fees or to request a Good Faith Estimate.

For more information about your rights, visit www.cms.gov/nosurprises.

Your privacy matters to us. At Three Birds Counseling, we understand that information about your health, personal experiences, and mental health care is sensitive, and we are committed to protecting it.

We maintain records of the care and services you receive to support quality treatment and meet applicable legal requirements.
